Isotretinoin 10 mg Capsules: Starting Treatment and What the First Weeks Look Like

The 10 mg capsule is the strength many people meet at the start of an isotretinoin course. Specialists often begin at a low daily amount, check how your skin and blood tests respond, and then adjust. If you have been prescribed isotretinoin 10 mg, you are probably wondering what happens before the first capsule, what the early weeks feel like, and what you can do to make treatment easier.

This guide walks through the start of treatment in order. It is general education and does not replace your specialist’s instructions.

Prescription-only medicine with strict controls. In the UK, oral isotretinoin can only be started by, or under the supervision of, a consultant dermatologist, and is supplied only against a valid prescription. Women who could become pregnant must follow the Pregnancy Prevention Programme. We cannot supply it without a valid prescription.

Before Your First Capsule: The Starting Checklist

Isotretinoin is not started on the day of your first appointment in most cases. Expect a series of checks first.

  1. Specialist assessment. Your dermatologist confirms that your acne is severe or resistant and that isotretinoin is appropriate. They will ask about previous treatments, medical history, mental health and other medicines.
  2. Baseline blood tests. Usually liver function and blood fats (cholesterol and triglycerides), and sometimes blood sugar.
  3. Pregnancy prevention steps, if relevant. A negative pregnancy test, at least one month of effective contraception beforehand, and a signed acknowledgement of the risks.
  4. Weight check. Your dose is calculated by body weight, and the target is a total amount over the full course.
  5. Information and consent. You should receive a patient leaflet and a patient card explaining the risks.

The Pregnancy Prevention Programme in Practice

The risk of serious birth defects is why this programme exists. For anyone who could become pregnant, it generally means:

  • A pregnancy test before the first prescription, then every month during treatment, and about five weeks after the last capsule
  • Effective contraception for at least one month before starting, throughout treatment and for one month after stopping
  • Short prescription supplies (up to 30 days), collected within a limited time of the test
  • No pregnancy during treatment or for one month afterwards

If you miss a period, think you may be pregnant, or have unprotected sex, stop the capsules and contact your doctor straight away. The MHRA’s isotretinoin guidance explains the current requirements.

Week by Week: What the First Month Often Looks Like

Period What many people notice
Week 1 Lips begin to feel dry; skin may feel tighter
Weeks 2 to 4 Dryness peaks for many; some people see a temporary acne flare
Around 4 weeks Follow-up blood tests and review; your dose may be increased if you are tolerating it well
Weeks 6 to 8 Many people see new spots forming less often
Month 3 onward Skin usually continues improving; monitoring continues at intervals

Everyone responds differently, so this is only a rough guide. Do not judge the medicine in the first few weeks, and do not stop because of the early flare without speaking to your specialist.

The early flare

A temporary worsening of acne in the first weeks is common and does not mean the treatment is failing. It usually settles as treatment continues. Do not pick or squeeze spots, as skin is more fragile and can scar. If the flare is severe, contact your specialist.

How to Take Isotretinoin 10 mg Capsules

  1. Follow your label exactly for the number of capsules and how often.
  2. Take them with a meal or a drink of milk. The medicine is fat-soluble, and food improves absorption.
  3. Swallow whole with water. Do not chew or open capsules.
  4. Take at the same time each day.
  5. Keep all appointments and blood tests.

If you miss a dose

Take it as soon as you remember, with food. If the next dose is nearly due, skip the missed one. Never double up.

A Starter Skincare Routine for the First Weeks

Dryness is the most common effect, and a simple routine eases it.

  • Cleanse gently with a mild, fragrance-free wash, once or twice a day, using lukewarm water.
  • Moisturise several times a day with an emollient designed for dry or sensitive skin.
  • Lip balm: apply frequently; keep one at home, in your bag and by your bed.
  • Sunscreen: SPF 30 or higher daily, since skin burns more easily.
  • Eyes: lubricating drops; consider glasses on days when contact lenses are uncomfortable.
  • Nose: a simple nasal emollient if you get dryness or nosebleeds.
  • Avoid: harsh scrubs, strong exfoliating acids, retinoid creams and any product your specialist has not approved.

Other Common Early Effects

  • Muscle and joint aches, especially after exercise: ease off intense workouts if they flare
  • Headache
  • Tiredness
  • Hair thinning or brittle nails, usually mild and temporary
  • Sensitivity to sunlight

Mood and Mental Health

Low mood, depression, anxiety and, rarely, thoughts of self-harm have been reported in people taking isotretinoin. A clear link has not been proven, and severe acne can itself affect mental health. Tell your specialist about any history of depression or anxiety before you start, and let someone close to you know what to look out for.

If you feel persistently low or agitated, or have any thoughts of harming yourself, stop the capsules and contact your doctor straight away. If you are in immediate danger, call 999. You can call Samaritans free at any time on 116 123.

Blood Tests and Dose Review

Isotretinoin can raise blood fats and liver enzymes. Doctors usually repeat tests about one month after starting and then at intervals, often every three months. If results are normal and side effects are manageable, the dose is often increased from the starting amount towards the target for your weight. The 10 mg capsule is often combined with other strengths to build that dose.

What to Avoid

  • Vitamin A supplements and high-dose multivitamins
  • Tetracycline-type antibiotics, such as doxycycline, lymecycline and minocycline
  • Waxing, laser treatment, dermabrasion and chemical peels during treatment and for about six months afterwards, unless your specialist advises otherwise
  • Blood donation during treatment and for at least one month afterwards
  • Excessive alcohol

When to Get Help Quickly

  • Severe headache with vomiting or vision changes
  • Yellowing of the skin or eyes, dark urine or severe abdominal pain
  • Severe stomach pain, bloody diarrhoea or painful swallowing
  • Persistent low mood, agitation or thoughts of self-harm
  • Widespread rash, blistering or swelling of the face or throat
  • Marked thirst or frequent urination
  • Sudden vision loss or poor night vision

Who Isotretinoin Is Not Suitable For

  • Anyone who is pregnant or breastfeeding, or who cannot follow the Pregnancy Prevention Programme
  • People with significant liver disease
  • People with very high blood fats
  • People taking vitamin A supplements or tetracycline antibiotics, unless advised by a specialist
  • Anyone allergic to isotretinoin, or to ingredients such as soya or peanut (check the leaflet)

How 10 mg Fits With Other Strengths

The 10 mg capsule is a building block. Your prescriber may use it on its own at the start, or combine it with 5 mg, 20 mg or 40 mg capsules as your dose rises. Never mix strengths yourself or adjust the dose without advice.

Storage

  • Keep in the original pack at room temperature, away from light and damp.
  • Keep out of reach of children.
  • Return unused capsules to a pharmacy; never share them.

Frequently Asked Questions

How long before I see results?

Many people notice improvement after four to eight weeks, with fuller results over several months. A course often lasts 16 to 24 weeks.

Why did my acne get worse at the start?

A temporary flare is common early on. It usually settles. Tell your specialist if it is severe.

Can I drink alcohol on isotretinoin?

It is best to avoid excess alcohol, as it can affect your liver and blood fats.

Can I wear contact lenses?

Dry eyes may make lenses uncomfortable. Use lubricating drops and consider glasses when needed.

Do I need blood tests?

Yes. They are a routine part of safe treatment.

Can I buy isotretinoin without seeing a dermatologist?

No. It must be started by, or under the supervision of, a consultant dermatologist and supplied on a valid prescription.

Is 10 mg a full dose?

It depends on your weight and plan. Many people take several capsules a day, and 10 mg is often a starting or building-block strength.

Can men take isotretinoin?

Yes. The pregnancy prevention rules do not apply to men, but the other safety and monitoring rules do.
